Mission

To identify and address the community's health and well-being by creating a network based on collaboration, coordination and partnering to improve the overall health of Linn County, Iowa.

Vision

A healthier community in which the well being of all who live, learn and work in Linn County is promoted and protected.

Philosophy

We believe that a healthier community is one in which all citizens play an active role in decisions that affect the health education and employment status of themselves and their neighbors, where those who are in need of assistance receive the support they require, and where the culture of the community promotes physical, emotional, social, mental, and spiritual wellness.

History

Healthy Linn Care Network (HLCN) was founded in 1989 with the goal of building partnerships to improve the health and well being of citizens in Linn County. Over 100 organizations joined together to conduct the Community Health Assessment in the mid-90’s. Since that time, HLCN has published two Community Health Plans (click on the link above to access the most recent Health Plan) - one in 1999 and, most recently, 2005. Charged with developing and maintaining collaborative networks among health care providers, businesses and community agencies, HLCN uses the Community Health Plan to identify and meet the wellness needs of all those who live, learn and work in Linn County.

Organization

An Executive Committee, consisting of John Brandt (Linn County Community Services), Shirley Holtey (St. Luke’s Hospital), Sister Susan O'Connor (Mercy Medical Center), Keith Erickson (Linn County Public Health) and Janet Ervin (Visiting Nurses Association) governs Healthy Linn Care Network. An Advisory Committee meets quarterly to support and advise HLCN staff as they implement strategies from the Community Health Plan, proactively address trends in health issues, and coordinate planning efforts.
