Improve the quality of life for all who
live, work and learn in Linn County
Healthy Linn Care Network was established to conduct a community health assessment
and develop the Community Health Plan. HLCN acts as a catalyst for community partnership
and collaboration to create and support programs that promote community health and
well being and foster the continuing development of partnerships among state and
local officials, health and human service providers, and community members. HLCN
and its workgroups follow a “healthy communities model” as developed by Tyler Norris
and his associates at Community Initiatives. Healthy community change is fostered
by:
Diverse
citizen participation & widespread community ownership
Building
capacity using local assets and resources
Program
benchmarks and outcome measurement
Focus
on “systems change”
Hundreds of community stakeholders came together to develop the
Community Health Plan under the direction of the Healthy Linn Care Network
and continue to participate in the different workgroups. Their goal is to improve
the quality of life for all who live, work and learn in Linn County.
The Community Health Plan provides the following:

A historical snapshot and analysis of community health problems identified by the
2004 Community Health Assessment. |

A model for broad-based, collaborative planning efforts aimed at securing grants
and improving community health care in the coming decades. |

A blueprint for implementing a community health system that fosters strong collaboratives
among organizations dedicated to acute care, public health, human services, and
education. |
